Privacy and data practices

Professional listing requests only. This directory is not a client portal, clinical service, or urgent-care channel. Do not submit client information, protected health information (PHI), treatment details, or urgent-care requests.
What the form collects
The form collects the request type; your relationship to the listing or request; provider name and credentials; practice or organization; email; a website or professional profile for additions and claims; optional region, modality, specialties, populations, payment, availability, and pronouns supplied by the provider, organization, or an authorized representative; optional notes; and a required acknowledgment of the review and privacy limits. Corrections, removals, error reports, and submission-deletion requests do not require a website or profile. A hidden form name and spam honeypot support Netlify form handling.
How we use it
Your email is used privately for response and listing-change confirmation and is not published. A submission never creates a listing automatically. Professional listing details may be considered for publication only after review. Pronouns and other identity fields are considered only when supplied by the provider, organization, or an authorized representative; they are never inferred. Other submitted details remain subject to review and direct confirmation. Notes are not published automatically.
Netlify and access
Netlify hosts this static directory and receives/stores form submissions in the project’s Forms area. Andrew and any Netlify user he authorizes may access submissions. An email copy is routed to info@wdtherapy.com. The directory does not sell or rent form submissions. Netlify and the email provider may retain technical logs or backups under their own policies, so this site cannot promise immediate or irreversible deletion from every system.
Correction, removal, and deletion
Use the form to request a correction, removal, or deletion. Select Update my listing, Report an error in a listing, Remove my listing, or Request deletion of my submission as appropriate. Removal requests are honored within five business days without requiring a reason. Deleting a previous form submission does not remove a published listing unless listing removal is also requested. Raw form submissions are targeted for deletion within 90 days after the request is resolved, with only the minimum operational record retained to document the action.
Analytics
We use Google Analytics to measure overall directory traffic.
