Add, update, claim, or remove a listing

This form accepts professional listing requests and good-faith corrections about mental-health, neurodivergence, assessment, and related referral resources. Submitting a request does not create a listing or guarantee publication, and inclusion is not an endorsement, verification, or recommendation. A person reviews every request before anything is added or changed. Addition, update, and claim requests may be applied, held for clarification, or declined. Corrections are reviewed before changes are made. Removal requests are honored within five business days without requiring a reason. Requests to delete a previous form submission do not remove a published listing unless you also request listing removal. A website or professional profile is required for additions and claims, but not for corrections, removals, or submission-deletion requests.

Before submitting: your email is used privately for response and listing-change confirmation. Do not submit client names, client contact information, health or treatment information, PHI, or urgent-care requests. Read the privacy notice and request correction, removal, or deletion.
